Instructions for using the KSVDL Rabies FAVN Electronic Submission Form

1. Access the electronic submission form
2. Enter all information in the designated areas
3. Review all information to confirm accuracy
4. Acknowledge signature requirement
5. Submit
6. Confirmation pop-up will ask if "You really want to submit?" by selecting Confirm.
7. Confirm order by selected "CLICK HERE" as shown below.

8. Submission form to include with sample for FAVN testing will be automatically generated as shown below.
   • 
9. Print the FAVN REPORT FORM
10. Make sure the Signature of Veterinarian is acquired.
11. Ship sample and form to:
