How to Complete the FAVN Submission Form Using the Online Portal

The FAVN online submission portal allows owners to quickly meet travel health requirements to transport animals from one country to another. This online submission will be completed by the veterinary clinic facilitating the blood collection.

Before you begin:

Your clinic will need to fill out the form using your KSVDL client account number. If your clinic does not have an account, please contact KSVDL Client Care at 1-866-512-5650 for assistance or utilize the New Client Registration Form . The client account number is associated with the name of the veterinarian as well as email address.

Starting the Online FAVN Submission Form

  1. Enter your client (clinic) information including account number, clinic name, veterinarian name and email address.

  2. Use the drop-down menu to select the destination(s) of travel
  3. List the animal owner’s first and last names.
    • If the animal is traveling to Hawaii, email address of the owner is also required

Entering and Saving Patient Information

  1. Click “Add Patient” button at the bottom of the form

  2. Enter both the animal ID and microchip number for each animal entry
    • The microchip number is entered twice on the form to avoid data errors
  3. Write the microchip number on the sample tube and verify that it matches the number on the form.
    • Samples should be sent in 12x75mm test tubes.
    • To avoid delays during sample processing, do not use microcentrifuge tubes, or any tubes smaller than 12x75mm or larger than 16x100mm.
  4. The serum draw date must be the day of submission or any day within the previous six months from the date the sample is being submitted.
  5. Select the species, breed, sex and age of the animal
  6. To facilitate the best interpretation of your results, please provide the last three vaccination dates, preferably from newest to oldest.
  7. To add another patient, click the “Add Another Patient” button. The information from the first patient is then saved and you will be directed to a new form to add another patient.
  8. Once all of the patients have been added, click “Save” and then “Submit.”
  9. An email notification will be sent to the email address provided. The email will contain a link which allows the client to make edits to the form if needed.

Signing and Submitting the Form and Sample

  1. Electronically sign the form
  2. Acknowledge all statements on the form, including the $112 fee for any information that needs adjusted by the lab after the test is finalized.
  3. Click "Pay/Submit"
  4. On the confirmation page, review the information entered for each animal on a PDF document. Information for multiple patients will be included on the same form.
    • If you do not see a “View PDF” button, you must pay to receive your submission form via email.
  5. A printed copy of this document MUST be included in the package that contains the specimens which will be shipped to the rabies laboratory.

Payment

  1. A submission email with be sent to the email address provided. The email will contain a link to edit the form content and a link to make payment for FAVN submission(s).
  2. Pay by credit card using the “Pay Now” button on the electronic form
  3. If you wish to pay later, the use the link found in the submission email.
  4. Enter credit card information and click pay

Test Results

    1. A final email with an attached PDF of the results will be sent to the email address associated with the account.
    2. This is your official FAVN electronic results report. You will NOT receive a hard copy of the test results.
    3. Each PDF result report will contain a unique QR code that will take you to a report verification page. Scan the QR code using the camera on your smartphone to verify your results.
    4. The URL of the report verification page will start with vet.k-state.edu/reportvalidation. Confirming the URL provides assurance the web page is KSVDL and the information on the web page accurately reflects the results of the test.
